After logging in mouse over the "Accounts" tab in the top menu and click "Personal Profile". 

After you complete the form you will receive 10 points which you can exchange for the sending of a report. We also request that all customers fill out this form; so that if there are any problems with your account, password or email address we will be able to use this information to assist you in getting back any lost information. This page cannot be seen by the public only by QCDIY staff.

If you need to upload a personal signature, please click the button "upload" we accept jpg, jpeg, gif, png and bmp formats.  The allowed maximum size for the signature photo is 200KB. Then click "Save" button.
